Personal supports for older adults who need higher levels of assistance to live independently. Visits are scheduled during the day, evening or overnight, with on-call support available 24/7 . Eligible recipients who qualify for Assisted Living Services are identified through assessment by Ontario Health at Home. Recipients must be able to live independently between visits and able to transfer independently, or with little support to qualify. The program is publicly funded, there is no cost to you.

Common tasks may include:

Personal care: Assisting with bathing, dressing, grooming, prompting medications, wellness checks and toileting
Homemaking: Light housekeeping, meal preparation, laundry, and shopping.
Care coordination: assistance managing care and accessing other support services.
