The ACT Team is a team treatment approach designed to provide comprehensive, community-based psychiatric treatment, rehabilitation, and support to persons with serious and persistent mental illness such as schizophrenia.Our ACT Team includes: psychiatry; nursing; occupational therapy, social work, addictions counselling, vocational counseling, and case management. We have an ACTT Stepped Care model that allows for transitional care for those clients who do not require the high intensity of services that ACTT provides but are not ready for discharge to case management. ACTT is an intensive program designed for individuals who have demonstrated high levels of system use, both healthcare and legal/justice.
