Description:
This clinic provides comprehensive, evidence-based care for patients who have been diagnosed with Congestive Heart Failure. The services are provided by an interprofessional team and include medical evaluation, prescribed exercises, cardiac risk factor modification, education, counselling, behavioural interventions, and will emphasize healthy heart living.

Eligibility Criteria
Criteria:
Physician referral required.Referral Criteria: Patients must meet the following criteria1) At least two hospital visits for heart failure within the last year:(Please indicate the dates on the form)2) NYHA Class III-IV CHFPatients will be considered on an individual basis if they have had one admission *for heart failure* within the last year and meet one or more of the following criteria: (Please indicate on the form which apply):- Sub-optimal drug therapy- Advanced heart failure (i.e. recurrent ER visits and/or frequent hospital admissions for heart failure)
