Description:

LEGHO is a 4-6 week program that offers navigation and community support services for people who have been recently discharged from hospital. This program covers the cost for people to receive services including frozen meals, rides to medical appointments, support with iADLs (i.e. light housekeeping, laundry, grocery shopping), home safety assessments, phone check-ins, etc. The program also offers a flex fund which provides discretionary funding for essential supplies and equipment.

Eligibility Criteria

Criteria:

Client was discharged from hospital within the past 7 days
Client is 60+ or is an adult at risk of visiting ED again in the next two months.
Client is ALC, medically stable and ambulatory.
Client is not currently living in LTC or retirement home.
Client must require two or more services to be eligible for LEGHO. (Note: This referral is generally not intended for mental health, addiction, or substance use support.)
