Description:
Provides dementia specific adult day centre services. An integral aspect of the D.A.Y. Centre model of care is ongoing caregiver support from the Social Work Program.
Daily hot meal and two snacks provided.

Eligibility Criteria
Criteria:
- Has been diagnosed with Alzheimer’s disease or other dementias
- Lives within the York Region
- Meets CCAC (Community Care Access Centre’s) eligibility criteria
