A custom care plan is created for each individual. A package of essential supportive services, in-home and community-based, is coordinated by a care manager who monitors the senior’s needs on a regular basis.

Criteria:
We assist low-income homebound seniors in Orange and Seminoles Counties with daily activities that have become difficult due to declining mobility and health. These individuals are at risk of nursing home placement because they are struggling at home without access to supportive services.
