Description:

Supports adults who have an intellectual disability and a psychiatric diagnosis.
RDDCT Consultation: Multidisciplinary assessment/consultation with recommendations directed to those involved in clients care. No follow-up or long term care involvement;
FACTT DD: Continuum of treatment from very intensive during periods of crisis and acute illness to less intensive during periods of stability. Provides continuity of service to clients and caregivers as client changes levels of intensity with the same team supporting them. A recovery based model with the goal of addressing gaps in current continuum of supports and services for the DD population, i.e. crisis response, increased case management capacity, support to housing providers, linkage to social recreation, vocational opportunities.

Other Criteria:
Maximum Age: 65 yrs
Minimum Age: 18 yrs
Criteria:
  • Documented diagnosis of a mental illness and a documented intellectual disability and adaptive functioning impairment, substantiated by intellectual assessment results, completed by a psychologist that are at the 1st percentile and below. Our DD psychologists must review this assessment to determine eligibility.
