Description:
Roger's House is about living life to the fullest. The experience of a "home away from home" fosters freedom, security, pleasure, and quality of family life. Children, youth and their families living with a progressive life limiting illness are entitled to access comprehensive palliative care services. This timely, holistic, patient and family-focused, culturally sensitive care is enhanced by the experience of Roger's House.

Eligibility Criteria
Criteria:
1. Referral to CHEO Pediatric Palliative Care Outreach team
2. Children/youth up to their 19th birthday
3. Ontario resident with valid OHIP coverage
4. Child/youth has life-limiting illness with progressive decline in health status
5. Family and child/youth must be willing to confront the challenging issues around a child's/youth's illness and death with the help of the interdisciplinary team
An individual plan of care (care directives) will be developed with the child and family's needs in mind
