Description:
The Community Alzheimer Program offers a variety of programs and services to support the needs of persons living with dementia, their families and care partners. Participants attending the program have the opportunity to engage in meaningful activities, develop relationships, and use their skills and talents in significant ways.
Current Service Offerings (during COVID-19 pandemic):
- Operating 6 days/week (Monday - Saturday)
- Recreation and social activities on an individual or group basis.
- Nutritious lunch and snacks
- Assistance is available for those who require support with meals, personal care and medication (including diabetes)

Eligibility Criteria
Criteria:
- Person has a diagnosis of dementia.
- Physically and behaviourally stable, and does not require 1:1 ongoing support from staff.
- Able to transfer and receive care with a maximum one-person assist.
- Able to follow all covid-19 protocols in place, including donning a mask when required, maintaining physical distance from other participants, and participating in COVID-19 Screeners.
- Aware that Sunnyside is a non-smoking campus, and that smoking cannot be supported in the program.
- Able to provide an emergency contact who is available to pick the participant up from the Program within 45-minutes of a call, if the need arises (ie: Participant becomes ill during the Program or cannot be safely supported in Day Program environment).
