Description:

Supportive Housing is for individuals with a physical disability whose needs cannot be met on a scheduled basis in the community. The services provided are limited to Attendant Care Staff (PSWs) and Homemakers who assist with personal care and activities of daily living (ie: Grocery Shopping, Meal Preparation, and Cleaning). Individuals must require personal care services. The individual rents their own apartment, pays their own utilities, furnishes their own apartment and manages their own further medical and support needs. This program is based on the Independent Living Philosophy and dates in structure to it's establishment in the 1980s. The Supportive Housing program operates within 3 PHARA facilities in North Bay.

Maximum Age: 150 yrs
Minimum Age: 16 yrs
Criteria:
Individuals aged 16 and older, with a valid Ontario Health card, a physical disability, requiring personal care services which cannot be met on a scheduled basis in the community, and able to self-manage their home.
