Description:
Program for pregnant parents, their partners, and families with young children from birth to transition to school age. Public Health Nurses and Family Home Visitors can provide support, resources, and connect families with community programs to help give children the best start in life.

Eligibility Criteria
Criteria:
- Client is pregnant or recently had a baby or is parenting a pre-schooler (up to age 6).
- Client is a legal guardian of the child(ren) and is actively parenting the child(ren) in the family’s place of residence.
