PFHT Health Clinic provides care to sudden and infrequent health conditions and illnesses. The clinic can only provide care to those without a family doctor or other primary care provider. By completing this form, it will alert staff at the clinic that an appointment is required. This form does not guarantee an appointment. This form is only to be used by New Canadian Centre staff.
