Description:
The Assertive Community Treatment Team is an intensive community based, client-centred, recovery-focused mental health service for people with serious and persistent mental health illnesses. The multi-disciplinary team operates as a "stand alone" service offering support and assistance in all aspects of daily living such as housing, vocational, financial, legal, psychiatric, and medical issues. Team members work with clients to develop and implement client-centered recovery and rehabilitation care plans ensuring clients access the appropriate level of services needed.

Criteria:
- Presenting with severe and persistent mental disorders such as schizophrenia, severe bi-polar disorders or other psychotic disorders.
- Having difficulty engaging with other mental health services.
- Confronted with significant functioning difficulties related to housing, social interactions, general life skills, schooling and/or employment.
