Helps adults without immediate support to arrive home safely upon transitioning from hospital, and receive follow-up services at home.
ONE CARE arranges services such as transportation, frozen meals and HAL attendant.
The attendant will meet the client at the hospital or at home.
The attendant can stay for up to four hours with the client.
Safety assessment of the home is completed.
Groceries or prescriptions can be picked up by the attendant.
The attendant can assist with, light meal prep housekeeping or laundry.
A complimentary meal (Frozen Meals on Wheels) is provided for each client.
Once the client is settled at home they will receive a follow up call within 24 to 48 hours.
