LEGHO provides access to essential community services for up to 6 weeks after hospital discharge, ensuring clients regain their strength and independence. A dedicated Care Planner conducts an assessment, provides weekly follow-ups, and connects clients with additional services to support recovery.
Core services include:
- Care Planning & Wellness Checks – Regular follow-ups and intensive care planning, starting within 72 hours of discharge
- Meals on Wheels – Nutritious meals delivered to support recovery
- Transportation – Rides home from the hospital and to two follow-up medical appointments
- Home Help – Light housekeeping meal prep and laundry
- Caregiver Support – Resources and assistance for family caregivers
