Service Details
Description:
A multi-component group based falls prevention program developed according to best practices in falls prevention and led by the geriatric outpatient rehabilitation service. Classes are led by the Physiotherapist, Occupational Therapist, Reactivation Worker and the Care of the Elderly Physician. The program runs for 12 weeks and consists of a 1 hour exercise component followed by a 30 minute educational component twice weekly.

65 yrs
Criteria:
- Older adults, 65 years of age or over, whose health, dignity and independence are at risk due to recent functional decline or impairment, falls or increased risk of falls, activity limitations, or de-conditioning as a result of an acute or sub-acute health condition, pain, injury, hospitalization or surgical procedure.
- Patient has been identified as having restorative potential:
The patient’s condition is likely to undergo functional improvement and benefit from interdisciplinary rehabilitative care or s/he requires rehabilitative care to prevent functional decline and
The patient is able, and willing to participate in and benefit from rehabilitative care; and
The patient has identified goals that are specific, measurable, realistic and timely. - Patients must be medically stable
- Patients must be living in the community (not long term care)
