Description:

A Geriatric Assessor and/or SGS physician travels to these communities several times per year to provide geriatric medicine consultation.



The goal of the Regional Outreach Geriatric Medicine Service is to reduce the burden of disability of older adults by detecting and treating reversible conditions, recommending optimal patient-centred care, and managing multiple co-existing chronic conditions. Every patient referred will receive a Comprehensive Geriatric Assessment (CGA) by the geriatrician/COE physician.


Eligibility Criteria
Other Criteria:
Minimum Age: 65 yrs
Criteria:
  • Multiple complex medical and psycho-social problems and/or
  • A recent unexplained decline in health and/or level of function and/or
  • Geriatric syndromes (incontinence, falls, pressure ulcers, delirium, cognition, polypharmacy, functional decline)
  • Living more than 1 hour from a geriatric medicine clinic
  • Cognitive, physical, transportation or frailty barriers do not allow the patient access to the outpatient clinic.
  • Need for a home safety assessment to due to identified risk on the referral.
