Our Board Certified Geriatric Pharmacist is available to assist patients living in the community to stay in the home of their choice for as long as possible. We are able to provide to assess medications through a lens of geriatric safety. Where necessary we can assist with deprescribing using evidence-based guidelines. We are also able to provide home assessments and make recommendations to enhance safety.
