Description:
Case Management is a partnership established with our clients who are affected by addictions or mental health challenges. We work with our clients to coordinate their areas of daily living to obtain food, shelter, income and linking them with our services and those offered within the community. It is our goal to foster the skills necessary for clients to live independent lives. This program also offers supportive counselling for clients experiencing mild to moderate mental health and substance use health challenges.

Criteria:
The Case Management and Supportive Counselling Program is designed for individuals experiencing mild to moderate mental health challenges, including depression, anxiety, and stress-related concerns, as well as those struggling with substance use and concurrent mental health conditions. This program is best suited for clients who can engage in goal-oriented support and benefit from structured guidance, resource coordination, and coping strategies. It is not appropriate for individuals with developmental challenges or those requiring a high level of support. Through personalized case management and supportive counselling, this program helps clients enhance their well-being, access relevant resources, and build the skills needed to improve their stability and quality of life.
