Dementia System Navigation: When a diagnosis of dementia is first made or throughout the journey, individuals and families may feel overwhelmed and be unsure of where to get support.
The First Link Care Navigator provides client-centred care planning and system navigation to individuals living with dementia and their care partners throughout Waterloo-Wellington.
The Navigator assesses the social, emotional, and educational needs of persons with dementia and care partners in order to develop and implement coordinated plans of support, making referrals and providing consistent follow-up as required. Services include:
-Initial individual/family clinical assessment
-Care planning and system navigation
-Referrals to Alzheimer Society programs and external community services
To reduce your client’s wait-time, please share the New Client Registration form with your client at the time of the referral.
