Description:
The team works with patients and their families to set achievable goals, which are based on individual needs, and monitored on a daily basis. Through daily therapy and active participation patients are able to track their progress toward these goals.

Cost to Client:
There may be some costs, such as an assistive medical device, that are not covered by OHIP when a patient is receiving rehabilitation services and care. These costs are the responsibility of the patient and their family.
Eligibility Criteria
Criteria:
- Patients who are medically stable, and therefore no longer require an acute care hospital bed. - They may include, but are not limited to, individuals who have experienced a stroke, a surgical procedure such as a hip replacement, cardiac, pulmonary or other medical conditions.
