Description:
The Regional Priority One Care Fund is intended as a supplemental funding source for agencies who are supporting a priority 1 ALHRS client transitioning back into their homes. The referral is made to the Home and Community Support Innovation Centre, which will validate that the client meets the eligibility criteria and approve the agency to use the funding model.

Eligibility Criteria
Criteria:
- Currently in Hospital, rated as a Priority 1, and classified as Alternate Level of Care
- Meets all ALHRS Eligibility Criteria
- Agency does not have a base funding slot to support client
- Patient is an insured person under the Health Insurance Act
- Patient is 60+ years or 65+ years for designated campus settings - Finlandia; Huron Lodge; Ontario Finnish Rest Home Association; Westwind
- Patient currently resides in, or has made application to reside in, a geographic service area where assisted living services are available. Patient does not reside in a retirement home (independent living apartment buildings excluded).
- Patient requires access to personal support services on a 24-hour basis and their personal support needs cannot be met solely on a scheduled visit basis. However, the patient does not require immediate or 24-hour availability of nursing care or other professional services.
- Patient requires personal support services to be delivered in a frequent, urgent and intense manner as below:
a) Frequent: Patient has needs where intermittent visits through the day may be necessary
b) Urgent: Patient has concerns/needs that warrant a prompt response that cannot wait to be scheduled;
c) Intense: Patient’s condition or predicament demands direct in-person care/attention from staff to address needs. - The patient meets Profile 1/2/3 criteria, OR the provider has determined that extraordinary circumstances exist that justify the provision of assisted living services to the patient
- Patient is able to remain safely at home between visits.
- Patient’s home has the physical features necessary to enable the service to be provided (e.g. home could accommodate mobility/transfer devices)
