Helps adults without immediate support to arrive home safely upon transitioning from hospital, and receive follow-up services at home. The service is up three hours on the day of transition and includes:
• Transportation home accompanied by a Personal Support Worker, including any stops such as: filling a medication, picking up light groceries or equipment.
• Help settling in at home from the Personal Support Worker
• Follow-up at home if not already connected to case management
