Requests may be placed on waitlist due to increased demand.
Service Details
Description:

Client Intervention & Assistance is a short-term service that supports and empowers individuals to live independently, with dignity, in their community. Services are available for vulnerable and at-risk seniors, persons with disabilities, and residents of care homes or supported living. All services are client directed, confidential, and delivered at no cost. This is a voluntary program and service users can withdraw from the program at any time. Please note that this is not a crisis service.



Services may include:



  • Personal assessment(s) at home or in the community

  • Information and referrals to community programs and services

  • Emotional supports & coping strategies

  • Advocacy for individuals’ needs and/or rights

  • Access to appropriate services and resources

Criteria:
Services are available to seniors 60 years of age or older and adults with disabilities who live in Chatham-Kent.
