As a Community Health Centre, we recognize that health is more than physical factors alone. In fact, health outcomes improve when people feel connected to their community, their cultures, and their environment.



We have a dual model of care to meet the needs of our community. We have a primary care clinic, and interdisciplinary care is important to how we work. Primary care providers are encouraged to practice social prescribing and communicate with community-facing staff ( Including care navigators) to know what resources are available in the community.



As well, we host free programs that are open to the community at large. Our programming aims to address social determinants of health. Some samples of our programming include, but are not limited to:
- Seniors care coordination: Care navigator on staff to help seniors and their families find support.
- Community meals and food security help.
- Media literacy and fraud awareness training
- Community climate action
- Emergency preparedness training
- Memory Cafes ( for people living with dementia and their caregivers)
- Dementia community education sessions
- Mental health group learning sessions
