Description:

The Darling Home for Kids supports children and their families living with a life-limiting or life-threatening illness.
Our support is provided with the purpose of assisting children and their families to live full, active, independent lives for as long as possible and when this is no longer possible, to help children and their families prepare and manage life closure and the dying process.

Eligibility Criteria
Other Criteria:
Maximum Age: 18 yrs
Criteria:
Children who have a progressive life-limiting illness or a major medical disease that demonstrates a progressive decline
Children who have a valid Ontario Health Insurance Plan (OHIP)
Children who are eligible to receive services from the Community Care Access Center (CCAC)
Families who have engaged in Advanced Care Planning discussions
In the case of End-of-Life Care, children must have a written Do Not Resuscitate Order (DNR)

In order to start the application process, please complete the attached application form.

Each completed application form will be reviewed by our admissions committee. Once your child's application has been reviewed, all service date bookings will be made through our Clinical Manager or Team Leader.
