To assist older adults who are socially isolated to remain active and connected to others, Through activity kit delivery to their home. Individuals registering for the program are asked to complete an initial questionnaire over the phone about their social/leisure interests. After completing the questionnaire participants receive an initial box of program activities and supplies that are tailored to their interests. Seasonally participants will receive a scheduled delivery with additional activities and a monthly program schedule. Participants are invited to participate in Zoom, telephone or in-person programs connected to the theme of the kits to support social connections.
