The Integrated Community Assessment Referral Team pro-actively identify high-risk seniors introduced or admitted to Hospital. High-risk seniors are offered additional support in the community to assist in the transition and recovery from hospital to home so that they can stay independent in their own home. ICART streamlines the provision of community-based services for identified clients.
iCART referrals accepted from HCCSS, Hospice Care and the following partnering hospitals:
• Brockville General Hospital (BGH)
• Lennox & Addington County General Hospital (LACGH)
• Kingston Health Science Centre (KHSC) - all locations
• Perth, Smiths Falls District Hospital (PSFDH) - all locations
• Providence Care
• Quinte Health Care (QHC) - all locations
- Age 60 + or adults with physical disabilities for ED referrals. No age requirements for CRISIS referrals.
- Client referred by HCCSS or Hospice Care or has been discharged from one of the above partnering hospitals
- Client is not active with SMILE or Veterans Affairs programs
- Patient requires supervision with one or more Activities of Daily Living (ADLs)
- Patient is not a resident of LTC
