The Integrated Community Assessment Referral Team (iCART) pro-actively identifies high-risk seniors who are introduced or admitted to hospital. These seniors are offered additional community-based supports to ease their transition and recovery from hospital to home, helping them to remain independent in their own homes.
iCART streamlines the coordination and provision of services across multiple providers, ensuring clients receive timely, seamless support tailored to their needs.
iCART referrals accepted from OH@H, Hospice Care and the following partnering hospitals:
• Brockville General Hospital (BGH)
• Kingston Health Science Centre (KHSC) - all locations
• KHSC@Home
• Lennox & Addington County General Hospital (LACGH)
• Perth, Smiths Falls District Hospital (PSFDH) - all locations
• Providence Care
• Quinte Health Care (QHC) - all locations
• Quinte@Home
- Client age 60+ for ICART-ED or age 18+ for ICART-ALC Crisis
- Client referred by OH@H, Hospice Care, or has been discharged from one of the above partnering hospitals
- Client is not active with SMILE or Veterans Affairs programs
- Patient requires supervision with one or more Activities of Daily Living (ADLs)
- Patient is not a resident of LTC
