Description:
Regional Care Planners (RCPs) provide in-home assessments for clients across the South East who request CSS support. An in-home environmental assessment is completed by RCP, along with the relevant intake package for the identified CSS agency that will be supporting this client. RCPs also develop the care plan and can refer for additional services that can benefit the client.

Eligibility Criteria
Criteria:
- Age 60 + or adults with physical disabilities
- Client requires in-home community support services
- Referrals must be sent from a Community Support Services Provider within the Ontario Health South-East Region or ICART referral.
