Description:

This is a multi-disciplinary service that provides intensive case management focusing on support, rehabilitation and treatment to individuals with severe and persistent mental health issues. The program helps people achieve their goals, improve their health and quality of life, and live as independently as possible. Team members include psychiatry, nursing, social work, occupational therapy, peer support, addictions, and therapeutic recreation.

Minimum Age: 16 yrs
Criteria:
Persons with serious mental illness with an axis 1 diagnosis.
