Description:

The Carefirst Community Cardiovascular Prevention & Rehabilitation Program is a half year program, and involves group education and discussions, and aerobic and resistance exercises. We assist patients in helping to make changes to their lifestyle and to build home exercise habits. Virtual options are also available.



The program is primarily designed for patients with coronary artery disease (such as heart attack) and diabetes, however may benefit if patients have any of the following:
• Heart attack or angina
• Heart Surgery (Angioplasty, Stent, bypass surgery, valve replacement or repair)
• TIA and minor Stroke


Therapy Services: Registered Dietitian, Registered Nurse, Social Work
Minor Medical Services: Blood Pressure
Languages: Cantonese, English, Mandarin
Cultural Affiliations: Chinese
Safety Features: Automated External Defibrillator (AED)
Activities:
Physical Activities: Strength Exercises, Stretching, Walking, Weight Training
Educational Activities: Guest Lectures, Health Topic Sessions, Nutrition Education, Speakers

Eligibility Criteria

Exclusion Criteria
We cannot accept patients who are:
• Medically unstable (i.e. uncontrolled HTN, uncontrolled arrhythmia, unstable angina)
• EF <40%
• Currently awaiting intervention (i.e. surgery, operations)
• Currently participating in another cardiovascular rehab program
• Significant cognitive impairment (i.e. unable to follow instructions independently or needing extensive support or guidance)
• Poor mobility (i.e. stroke affected, unable to ambulate independently despite mobility aid, wheelchair bound, paralysis, syncope, high falls risk)
• Non English or Chinese speaking
• Resident(s) of LTCH
