The Regional Care Planner (RCP) will visit the clients home and provide a variety of care coordination supports including in-home assessments, care plan development and eligibility determination for local community support services. Once a care plan has been developed, the RCP will help facilitate referrals and care plan updates to the appropriate local community agencies.
