Description:

Provides end of life care in a home like setting. Hospice Cornwall is a home to our clients when their own home is no longer possible or practical, making the last days of a person's life together with their family peaceful, comfortable and meaningful.
Hospice palliative care focuses on providing relief from distressing symptoms when the disease is no longer curable. Since the focus is on providing comfort measures, the role of invasive medical treatment is limited.

Eligibility Criteria
Criteria:
For residents of the City of Cornwall, Stormont, Dundas, and Glengarry, as well as Prescott and Russell Counties. Applicant must be in the final stages of life, with a prognosis of approximately three months or less. Must be followed by their own family physicians. Valid Ontario Health Card. Signed Do Not Resusitate Order. Eligibility for CCAC services. Exclusion: Wandering, unsafe behaviour, violent behaviour, chronic prognosis, desire for curative treatment
