Description:
Provides transportation service for older adults in need who are unable to use public transportation or private means. Provides transportation to medical appointment, cancer treatment and dialysis.

Minimum Age:
60 yrs
Criteria:
- Client resides in the Nipissing District and is a minimum of 60 years.
- Client lives at home or with a loved one (not in a long term care facility).
- Client is able to transfer in and out of the vehicle independently or may bring an escort to assist.
- Client is unable to access existing transportation options (Transit) and is not currently driving.
- Client has been notified that an Intake Assessment is required.
- Client is able to attend appointments independently or will have an escort to provide support.
