Description:
Provides time limited case management of up to 3 months, in home, community and office settings including supportive counselling, psycho-education, symptom management and coping skills, social and recreational activities, health care and social services navigation/support.Option for transfer to longer term case managment based on individual needs and treatment plan goals.
