Description:
Bilingual interprofessional outpatient geriatric rehabilitation program that aims to optimize the health and quality of life of seniors in our community.
The team works closely with the client and caregivers to set goals and implement a personalized therapeutic program that can last up to 8 weeks.
Clients typically participate in the program two half days each week.

65 yrs
Criteria:
- Client must have a physician referral
- Client has difficulty activities of daily living, such as bathing, meal preparation or problem-solving in emergencies, or cognitive issues, or caregivers are experiencing stress
