Description:
Community Outreach Support Workers assist and engage the Person Living with Dementia and their families. The Community Outreach Worker will support a variety of needs including planning and organizing various activities while providing safe, effective, competent care along with support and relief for the care partner. The Community Outreach Worker will work with you and your family to develop an appropriate support plan that will reflect the unique needs of both the Person Living with Dementia and their Care Partner.

Eligibility Criteria
Criteria:
Person Living with Dementia in the Moderate or Late Stage that would not be able to access an adult day program and has minimal or no other community services.
