Description:
The program provides social, recreational, intellectual and physical activities in a structured, supervised group setting. Throughout the day, clients are provided with refreshments, a hot nutritional meal, companionship and help with personal care needs such as medication reminders and toileting.

Eligibility Criteria
Criteria:
- Adults 18 years of age and up with a valid Ontario Health Card- Frail seniors and adults with Alzheimer's disease, Dementia, or another degenerative cognitive impairment.- Living in the City of Hamilton .

A referral & eligibility form can be completed from Adult Day Program supervisor or made through the Hamilton Community Care Access Centre (CCAC) at 905-523-8600 or if you are currently receiving CCAC services, please ask your Care Coordinator.
