Description:
St. Joseph's Adult Day Programs are for older adults, living in the community, alone or with family, who would benefit from a wellness-oriented program, including social recreational activities, monitoring and assessment in a supervised, therapeutic environment. The day program also provides caregiver relief, education for families and individuals, and support and advocacy. Program is NOT suitable for those:
- who require 2-person transfers
- who require constant 1:1 support
- who require the use of a mechanical lift
- who require feeding assistance

Available from: Transportation is NOT available through the day program. Other organizations include VON, Ride Well, private taxi's
Physically and behaviourally stable and does not require 1:1 ongoing support from staff.
Be able to independently feed themselves.
Can/will the individual participate in group-based programming?
Able to transfer and receive care with a maximum 1 person assist (unable to support the use of transfer boards).
Aware that SJHCG is a smoke free facility, and smoking cannot be supported while in the day program unless the individual can independently go off the property and return.
Able to provide an emergency contact who is able to pick up the participant from the program within 45 minutes of a call, if the need arises (ie. participant becomes ill or cannot be safely supported in the program).
Able to provide an email address for the participant or the caregiver to receive the referral confirmation (no longer able to provide telephone confirmation).
