Description:
Qualified attendant care staff provide assistance with activities of daily living in their place of residence, employment, community or educational environment. The services include, but are not limited to, assistance with personal hygiene, rising and retiring, meal preparation and toileting.
Cost to Client:
Services are funded through the Local Health Integration Network
Eligibility Criteria
Other Criteria:
Minimum Age: 16 yrs
Criteria:
Have a permanent physical disability which results in the inability to independently complete personal care routines; require assistance with personal care as a result of a physical disability; have the ability to self-direct his/her own care to the eligibility level for each service; be a resident of Ontario and insured under OHIP, and; not be in receipt of or expecting to be in receipt of insurance funds for provision of these services
