After Stroke is personalized stroke recovery program that can help you and your family navigate the path forward after a stroke. Our team listen, share their experiences, and help you find the support and information you need. Our expert coordinators work with you to understand your needs and set personalized recovery goals, connection with education, resources, and community-based programs that will help reach recovery goals, feel better, and build confidence.
Once you have submitted a service request, your local After Stroke coordinator will contact you. They will work with you to understand your needs and goals for stroke recovery, and develop a personalized plan to help you reach them. The plan is regularly reviewed and adjusted. They will help you connect to support, educational opportunities, programs, and other stroke survivors who have been in your shoes.
