Eligibility Criteria
Criteria:
Any person 18 years of age or older with a valid Ontario Health Card issued by the Ontario Ministry of Health and Long-Term Care. Additionally, this individual must be deemed eligible to apply for admission to a long term care home by the Community Care Access Centre.
