**Choose this service if you are booking for a patient under 12 years of age.** We encourage parents/guardians/caregivers to provide assistance for specimen collection. Please consider talking your child through the process prior to the appointment so they feel more at ease. Feel free to bring a small comfort item for your child.

Appointments are required for all laboratory testing.

**Special Considerations:**

**Fasting Bloodwork**
- Please consider if your appointment is fasting or non-fasting.
- *If you do not need to fast overnight, please consider booking an appointment after 10am.*

**Overnight Fasting is required for the following tests:**
- Glucose Fasting
- Glucose Tolerance Test GTT/75 GTT/OGT
Note: 50g Glucose for Gestational Diabetes does not require fasting.
- C-peptide
- Growth Hormone (GH)
- Bile Acids
- Insulin
- Urea breath test (requires 4 hours fasting)

**Tests that are not available on Fridays:**
Please do not book your appointment on a Friday if your requisition lists one of the following tests:
- Flow Cytometry
- Fragile X
- Hemochromatosis
- HLA B27
- Immunology Markers
- Quantiferon TB Gold
- Sirolimus
- Tacrolimus
- C3a and C4a Complement Level

**Tests that are not covered by OHIP:**
Note that some tests are not covered by OHIP and will require payment to Dynacare Laboratory (for example: urea breath test, vitamin D if not insured). Please contact the laboratory if you have any questions regarding coverage.

**Registration**
All patients requiring bloodwork will check in directly at the Laboratory Front Desk. Please do not go to Central Registration.

**Requisitions**
If your health care provider has given you a requisition for testing, you must bring it with you or upload the form when you book your appointment. If you are uploading your requisition, please only upload documents - screenshots or pictures of requisitions are not acceptable.

If your requisition is from a Naturopath: Dynacare will only accept those that are on a specific Dynacare requisition (not a Ministry of Health requisition) with a private account number from the NP as these are not covered by OHIP.
