The Timiskaming Community Paramedic Program extends the traditional role of paramedics beyond emergency response to include proactive healthcare services within the community. We aim to improve access to care, especially for vulnerable populations, by providing in-home medical assessments, chronic disease management, health education, and post-hospital discharge follow-ups.
