The program also connects patients with local resources and agencies to coordinate broader care. For those with ongoing health needs, remote monitoring is available to track key health indicators, helping to prevent crises. This program brings healthcare to individuals, empowering them to manage their health safely at home.

Eligibility Criteria
Criteria:
Client must meet at least one of the following:

• Has complex medical needs or one or more chronic conditions
• Frequently uses paramedic services or visits the emergency department
• Has difficulty managing health needs safely at home
• Is at risk of hospital admission
• Is experiencing homelessness or is at risk of becoming homeless
• Has mental health or substance use concerns impacting health or stability
