Description:
Day programs promote the inclusion of individuals with language, gender, cultural, racial, religious, physical, cognitive or health diversity to come together to socialize, share and enjoy each other's company while engaging in social and recreational activities. Programs are offered in safe and inclusive spaces that adhere to a Code of Conduct that promotes dignity and respect for all.

General Information:
* programs open Monday to Saturday
* clients may attend multiple days
* short waitlist- start within 1 to 2 weeks after an on-site tour
* variety of programs to meet different interests & needs
* 2 locations in Cambridge

Languages:
Individuals whose first language is not English are encouraged & welcome to attend.
> Portuguese speaking individuals usually attend Wednesday or Friday
>Francophone programming is available Tuesday & Thursday
> individuals who struggle with communication in English or French or live with aphasia successfully attend programs
> staff fluent in English Monday to Saturday
> staff fluent in French on Tuesday & Thursday
* Personal care:
> assistance for activities such as dressing, feeding, medication reminders, 1 person transfers, washroom assistance, personal hygiene care and mobility is available with a trained Personal Support Worker,

Cognition
>opportunities for those with mild to moderate cognitive decline due to dementia, Alzheimer Disease, Acquired Brain Injury to attend

Transportation
> assistance to arrange transportation for individuals not eligible for Grant River Transit MobilityPLUS or unable to provide their own ride. Additional fees apply.

Programs are not suited for individuals :
> with aggressive or exit seeking responsive expressions;
> requiring 2 person or mechanical lifts or transfers
> living with daily unscheduled severe bowel incontinence
> needing to sleep or lie down for a nap
> refusing or unable to engage and participate in activities outside their home
> with responsive expressions requiring 1 to 1 or intensive staff supports
> requiring medication administration

When programming is not suitable, staff support families to find alternative programming available in the community to better meet needs of each individual client and to connect with other community based supports and resources.

$24.06 - $31.50 per day; $8.00 transportation fee if needed. Subsidy Available. is available for individuals with low income at or below the federal Low Income Cut Off (LICO)
Eligibility Criteria

Minimum Age: 18 yrs
Criteria:
Any adult (18+) who is not able to attend other social and recreational programs offered in the community and meets one of the following criteria:
> has an age related dependency or frailty
> is living with a physical or mental impairment or chronic or degenerative health condition
> is experiencing depression, loneliness or social isolation
> requires assistance to remain independent and socially connected in the community
> lives with a caregiver or family member or is a senior caregiver who desires and needs some time away from their care giving role
AND
> lives in or around Cambridge or North Dumfries or has access to GRT Mobility PLUS services or will provide own transportation from Kitchener or Waterloo or surrounding areas
> will benefit from social interaction and stimulation in a supportive setting
> is willing and desires to attend and adult day service program

While the majority of participants are older adults over the age of 75, day programs are available for adults age 18 and over who need staff support or supervision to safely attend social recreational programming that is otherwise not available for them in the community.

How to Apply:
Referral through a variety of health care providers including but not limited to Ontario Health at Home 519-748-2222, any medical professional or community supports service providers. Family members, friends or potential participant may refer themselves directly 519-513-6677 An in-person referral visit to the main centre by client and/or caregiver(s) is required to provide information about program options, complete required paperwork and help match client to the best suited day program.
