Must be a resident of Kingston or have immediate family in area. Must have a valid Ontario health number issued in his/her name. Must be on the roster of the Access Centre for Kingston so that we can access the professional services such as the visiting nurses that they provide. Must be in the final stages of a terminal illness. We are looking at admission during the final 1-3 weeks of life or at 30% on the Palliative Performance Scale Admission to the Hospice House will be based on greatest need.Palliative patients in the hospital will be given the option of admission to the Hospice House if no one in the community is at the stage for admission. A "do not resuscitate "form will be signed as no heroic measures will be taken at end of life. Clients/family will have a personal interview with a representative of Hospice Kingston prior to admission, as to the expectations and services that Hospice provides.
